PUBLIC NOTICE: HIPAA Breach Notification. Pacific Ocean Pediatric would like to alert patients that on or around March 16, 2017, there was an office break-in which resulted in the theft of over 6,500 patient charts containing confidential patient information that was stored on a hard drive and locked in a safe in our office. Pacific Ocean Pediatric is working with law enforcement to identify the criminal and recover the information. The information stored in the stolen hard drive includes, but is not limited to, the following: names, addresses, phone numbers, birth dates, insurance information, as well as the entire charted medical history of the patient. Additionally, some parent Social Security Numbers were included in the files. Therefore, we urge our patients to closely monitor all accounts and credit information by contacting Experian, TransUnion, and Equifax.
